Why Are Hospitals Prescribing Video Games? The Rise of Digital Mental Health Tools

Can a Game Replace a Pill? The Surprising Role of Prescribed Play in Medicine
Video games have long been seen as escapist entertainment, associated with leisure or, at times, digital addiction. But in recent years, something radically different has emerged—clinically approved games prescribed as treatment tools. In 2020, the U.S. Food and Drug Administration (FDA) made headlines by clearing the first-ever prescription video game, EndeavorRx, for children with ADHD. Since then, the field of digital therapeutics has exploded, with researchers, developers, and mental health professionals converging to explore how video game mechanics can support psychological healing, neurological rehabilitation, and emotional regulation. Hospitals, once home to white coats and stethoscopes, are now increasingly collaborating with game developers to bring joy—and therapy—to the bedside.

What Is a Digital Therapeutic Game—and How Does It Work?
Digital therapeutics are software-based interventions that deliver evidence-based treatments through devices like tablets, smartphones, or VR headsets. In the case of prescription games, these tools are designed with specific neurocognitive or psychological outcomes in mind. Take EndeavorRx, for example: it’s not a traditional game with levels or scores but a cognitive training platform. Developed by Akili Interactive and backed by clinical trials, it targets neural systems associated with attention function through adaptive, challenge-based gameplay. Children navigate a spaceship, collecting items and dodging obstacles—all carefully calibrated to engage focus, impulse control, and working memory. The game adjusts difficulty in real time, using machine learning to keep players at the edge of their cognitive ability, just like physical therapy but for the brain.

How Video Games Affect Neurotransmitters and the Brain
So why are games such effective tools for mental health? The answer lies in their unique ability to engage neurotransmitter systems in ways that are both therapeutic and rewarding. Video games stimulate the brain’s dopaminergic reward system—releasing dopamine, which is linked to motivation and pleasure. Games that require goal-setting, problem-solving, and focused attention activate the prefrontal cortex, the area implicated in executive function. Additionally, some games can regulate the autonomic nervous system, lowering cortisol levels and heart rate variability—similar to what happens during meditation or biofeedback. Importantly, games also offer “safe stress”: challenging yet achievable tasks in a controlled setting. This fosters resilience and can help with anxiety disorders, PTSD, and even depression when used in a structured and supervised context.

FDA-Approved Game Therapies: What’s Available and What’s Coming
The FDA’s clearance of EndeavorRx marked a turning point, legitimizing game-based interventions in clinical treatment. Since then, companies like DeepWell Digital Therapeutics, MindMaze, and Litesprite have been developing games targeting everything from PTSD to anxiety to pain management. Litesprite’s game Sinasprite, for instance, offers a cartoon fox that teaches cognitive behavioral therapy (CBT) skills, mindfulness, and emotional regulation through quests and journaling. Another promising tool, SnowWorld, is a VR game used to distract burn victims during wound care, proven to reduce pain perception by as much as 50%. Clinical trials are underway to explore gaming solutions for substance abuse, Alzheimer’s disease, and even schizophrenia. Unlike generic wellness apps, these are developed under strict research protocols, with peer-reviewed studies validating their impact.

The Clinical Setting: How Hospitals Are Using Games in Practice
From pediatric clinics to veterans’ hospitals, video game interventions are being adopted into care protocols. Children who once resisted therapy sessions now look forward to “playing” their treatment. In adolescent psychiatric wards, games are used to develop emotional regulation skills, track behavioral triggers, and improve communication. In rehabilitation centers, stroke survivors use movement-based games to rebuild motor skills, while trauma patients engage with VR environments to gradually confront phobic stimuli. Some hospitals have even established “Digital Play Rooms” where supervised game sessions form part of a broader integrative care plan. These programs are particularly effective in rural or underserved communities where mental health specialists may be scarce—game-based therapy fills the gap, offering scalable, stigma-free interventions.

Healthy Play or Digital Dependency? Navigating the Risks
Despite the promise, prescribing video games also raises ethical and psychological concerns. Critics argue that it blurs the line between therapeutic intervention and screen overuse. Young patients, already prone to excessive screen time, may struggle to differentiate between structured therapeutic gaming and recreational bingeing. There’s also a risk of over-reliance—viewing the game as the solution rather than a tool in a broader treatment plan. That’s why most digital therapeutics include strict guidelines: time-limited usage (e.g., 25 minutes per day, 5 days per week), embedded parent controls, and clinician monitoring. Moreover, professionals stress the importance of integrating gaming interventions with traditional therapy methods—conversation, reflection, community, and supervision remain irreplaceable. The best use of digital tools is as a complement, not a substitute.

VR, AR, and the Future of Game-Based Mental Health
Beyond smartphone games, VR and AR are rapidly advancing the scope of digital mental health tools. Platforms like Psious, Oxford VR, and BehaVR offer immersive experiences tailored to treat phobias, social anxiety, and even schizophrenia. These tools use real-world exposure simulations, allowing patients to virtually confront their fears in a graded, controlled way—a technique rooted in exposure therapy. For example, someone with a fear of flying might virtually board a plane, complete with engine sounds and turbulence, all while guided by a virtual therapist. These scenarios reduce avoidance behaviors and help rewire fear responses. With the rise of wearable biometric tracking (e.g., HRV, EEG headbands), these platforms can soon adapt in real time to user stress levels, making treatments even more personalized and effective.

From Couch to Console: Can Adults Benefit Too?
Although much of the initial research has focused on pediatric populations, adult mental health is increasingly being addressed through game-based tools. For example, ReMission—originally developed for young cancer patients—has found applications in adult oncology care to reduce treatment anxiety. In the workplace, companies are experimenting with gamified stress-reduction apps as part of employee wellness programs. PTSD treatment among veterans is another frontier: VR environments are helping retrain trauma responses in safe, controlled settings. Even older adults are benefitting—games like NeuroRacer and Lumosity are being investigated for cognitive maintenance and early detection of memory decline. In all cases, the key is intentional use: the game is designed not for escape, but for structured engagement, insight, and healing.

The Therapist’s View: When Games Work—and When They Don’t
Many mental health professionals now receive training in integrating digital therapeutics into care. They emphasize the importance of selecting clinically validated games, establishing therapeutic context, and involving the patient in goal-setting. A therapist might review gameplay data to identify patterns, discuss emotional responses during challenging levels, or use game content as a metaphor in traditional talk therapy. However, they also note that not every patient is a good fit. Individuals with severe dissociation, screen addiction histories, or specific psychotic symptoms may require alternate modalities. Success lies in personalization—knowing when to prescribe a digital game and when to stick to analog methods. Ultimately, clinicians see game-based therapy not as competition, but as collaboration between psychology and technology.

Conclusion: Are Video Games the Future of Mental Healthcare?
Digital mental health tools are not just a trend—they represent a paradigm shift in how we understand, deliver, and democratize psychological care. Video games, when designed with scientific rigor and therapeutic intent, can do more than entertain: they can heal. They offer immersive, engaging, and scalable interventions for conditions as varied as ADHD, PTSD, chronic pain, and anxiety. But they must be used thoughtfully—monitored by professionals, grounded in research, and supplemented by human connection. As hospitals, schools, and families navigate this evolving landscape, one thing is clear: play has a powerful place in the healing process.
